PNCC services help expecting mothers or mothers of new infants to gain access to medical, social, educational, and other services related to the client's pregnancy.

How does Lily Rose Care Coordination assist with PNCC services?
Lily Rose Care Coordination assists pregnant women in obtaining prenatal care coordination and support needed in order to have a healthy pregnancy, as well as remain healthy and stable after pregnancy.

Who is eligible for Prenatal Care Coordination Services and for how long?
Wisconsin Medicaid PNCC services are available to Medicaid-eligible pregnant women with a high risk for adverse pregnancy outcomes during pregnancy through the first 60 days following delivery. After 61 days, eligibility transfers to the Child Care Coordination option, which covers supportive services for children for up to 7 years after enrollment.

What does the PNCC process with Lily Rose Care Coordination consist of?
Lily Rose Care Coordination follows State of Wisconsin prenatal care coordination procedures to ensure the health and wellness of pregnant mothers.
We will assess and verify eligibility, as well as measure specific needs through simple health assessments for mother and for family members if necessary.
We will assist pregnant mothers in their development of personal health plans, providing ongoing care coordination and providing health education and nutrition counseling support services through referrals or planned organization with professional partners.
Our Care Coordination model also consists of agency-wide value for education and we are willing to expand to support mothers whose care plans include education, job training, personal development. Whether the goal is to go to college, get a new job, get CBRF trained or CNA trained, or to develop entrepreneurial skills, we have in-house professionals whose backgrounds consist of and help fuel these additional goals.
